Community Alliance, offering an integrated health care approach and built on an array of mental health and primary care services, is seeking a Case Manager for our Integrated Care Clinic. Ideal candidates will have passion for working with vulnerable populations and comfortable in collaborating with other professionals to empower clients to achieve self-defined goals.
Schedule: Monday - Friday, 8:30 am - 5:00 pm. No weekends and No on-call.

The Case Manager in Community Alliance's integrated care clinic is  responsible for providing direct and collateral case management services which promote personal physical and mental health wellness and recovery for adult individuals with serious mental illness and co-morbid physical health conditions. An individual in this role contributes to the attainment of clinic goals and objectives, and is responsible for providing a consistent point of contact within the program which serves as the integrated health home for persons served.


Duties Include:
  • Provide case management sources and follow up with client needs related to community resource needs, including financial entitlement, medication assistance and other entitlement programs.
  • Assist persons served in identifying, accessing, and utilizing primary and specialty health services.
  • Actively assist persons served in effective communication and self-advocacy with primary and behavioral health providers.
  • Assist individuals in making and remembering appointments, arrange transportation, and keep scheduled appointments for services.
  • Provide individualized, strength-based support, education, coaching, information and advocacy services.
  • Continuously develop and utilize knowledge, skills, and competencies necessary to perform position responsibilities in a high quality fashion and consistent with organizational, accreditation, and professional standards.

Minimum Qualifications:
  • Bachelor’s Degree in Social Work, Rehabilitation, Education or other equivalent area of study from an accredited academic institution.
  • Demonstrated knowledge of characteristics and needs of persons with mental illness.
  • Demonstrated knowledge and skills in mental health rehabilitation and recovery principles.
  • Ability and willingness to work varied hours, including evening and weekend hours as necessary.
  • Ability to transport self to and from community sites to perform position responsibilities including possession of valid driving license, good driving record, and reliable transportation, or the ability to arrange for such transportation on a regular basis.
  • Ability to maintain effective working relationships with individuals of diverse backgrounds including persons with mental illness, behavioral health and physical healthcare professionals, family members, social service providers, and public at large.
